A social worker can play an important role in helping an elderly person stay safe, supported, and connected to the right resources. Many seniors and families feel overwhelmed when they are trying to manage healthcare needs, home care, housing, insurance, financial concerns, or long-term care decisions. A social worker helps guide families through these challenges.


For an elderly person, a social worker may help assess what support is needed at home. This can include help with care planning, transportation, meals, caregiver services, medical appointments, safety concerns, or emotional support. They may also connect seniors with community programs, benefits, and services that can improve quality of life.


Social workers often assist with hospital discharge planning. If a senior is leaving the hospital after an illness, surgery, or injury, the social worker may help coordinate home health care, rehabilitation, medical equipment, or placement in a skilled nursing facility if needed.
