Does Medicare Pay for an At-Home Caregiver?

Families often need help when an older adult can no longer manage bathing, dressing, mobility, meals, or household tasks independently. A common question is, does Medicare pay for an at-home caregiver? Medicare may cover limited home health aide assistance when specific medical requirements are met, but it generally does not pay for ongoing custodial care, companionship, or full-time supervision.
When Medicare May Cover Caregiver Help at Home
Original Medicare may cover part-time or intermittent home health aide services when the patient is homebound and also needs qualifying skilled care, such as intermittent skilled nursing, physical therapy, speech-language pathology, or continued occupational therapy. A healthcare provider must order the care, and a Medicare-certified home health agency must provide it.
Covered home health aide services may include help with bathing, grooming, dressing, walking, feeding, or changing bed linens. However, Medicare generally covers this personal assistance only while the patient is also receiving qualifying skilled home health services.
Medicare does not normally pay for 24-hour care at home, meal delivery, shopping, housekeeping, laundry, or personal care when those are the only services needed. It also does not usually cover a companion who provides supervision or social support without a medical component.
Eligible patients generally pay nothing for Medicare-approved home health services. Durable medical equipment, such as a walker or wheelchair, may involve separate Part B cost-sharing. Medicare Advantage members should contact their plan because provider networks, authorization rules, and additional benefits can vary.
Before hiring private help, ask the senior’s healthcare provider whether skilled home health care is medically necessary. You can also contact the Medicare plan and request a clear explanation of covered services and possible out-of-pocket costs.
